In de nabije toekomst hoeven vrouwen voor onderzoek naar het humaan papillomavirus (HPV), de verwekker van baarmoederhalskanker, niet meer naar een arts. Er is namelijk een zelfstandige screening op HPV op komst door middel van een thuistest. Een onlangs gepubliceerde studie van onderzoekers van het VU medisch centrum, Amsterdam, en Radboudumc, Nijmegen, in The Lancet Oncology liet zien dat, bij vrouwen met een HPV-positieve thuistestuitslag, een nieuwe moleculaire test even betrouwbaar is als de ‘klassieke’ methode met een aanvullend uitstrijkje uitgevoerd door een arts.
Vrouwen met HPV worden nu verwezen naar een arts voor een uitstrijkje (triagetest) om te bepalen of er afwijkende cellen zijn. Als er afwijkende cellen worden aangetroffen, is de kans op voorstadia van baarmoederhalskanker (cervicale intra-epitheliale neoplasie graad 2 en ernstiger; CIN2+) hoog en worden de vrouwen verwezen naar de gynaecoloog.
Dat het aanbieden van de HPV-thuistest aan niet-deelneemsters van het bevolkingsonderzoek baarmoederhalskanker (BVO BMHK) leidt tot een stijging van deelname aan het BVO is reeds aangetoond.1,2 Door vrouwen met een HPV-positieve thuistest een triagetest direct op het thuistestmateriaal aan te bieden, kan een extra bezoek aan de huisarts voor een additioneel uitstrijkje worden bespaard. In de onlangs verschenen studie in The Lancet Oncology is gekeken of moleculaire triage met DNA-methylatie direct op thuistestmateriaal even betrouwbaar is voor CIN2+-detectie als cytologie op een extra uitstrijkje afgenomen door een arts.3
PROHTECT-3
In de gerandomiseerde gecontroleerde studie (PROHTECT-3) zijn 46.001 vrouwen (33-63 jaar), die niet hebben deelgenomen aan het BVO BMHK in 2007, uitgenodigd om een thuistest af te nemen. 12.819 vrouwen retourneerden een thuistest aan het laboratorium waarvan 1.038 HPV-positief werden getest tussen 1 november 2010 en 31 december 2011. Van de vrouwen met een HPV-positieve thuistest zijn, na exclusie, 509 gerandomiseerd in de cytologiegroep en 515 in de moleculaire-triagegroep. In de cytologie-triagegroep werden vrouwen verwezen naar de huisarts voor cytologie op een extra uitstrijkje. In de moleculaire groep werd direct op het thuistestmateriaal een DNA-methylatieanalyse van de genen MAL en miR124-2 verricht.4 Vrouwen met een positieve triagetest (abnormale cytologie; ≥PAP2, of een positieve methylatietest) werden doorgestuurd naar de gynaecoloog voor colposcopie met biopten. Na zes maanden werden HPV-positieve vrouwen met een negatieve triagetest uitgenodigd voor een uitstrijkje en HPV-test; bij een afwijkend uitstrijkje en/of positieve HPV-test werden vrouwen verwezen naar de gynaecoloog.
CIN2+-detectie in de moleculaire-triagegroep was gelijk aan die van de cytologie-triagegroep; 90 (17%) van de 515 vrouwen versus 75 (15%) van de 509 vrouwen (relatieve risico 1,19; 95% BI 0,90-1,57). In de moleculaire-triagegroep werd vaker doorverwezen naar colposcopie (284 vrouwen; 55%) dan in de cytologie-triagegroep (149 vrouwen; 29%; p<0,0001). Daar staat tegenover dat de gemiddelde tijdsduur om tot een diagnose van CIN2+ te komen korter was in de moleculaire-triagegroep (96 dagen; spreiding 44-101) dan in de cytologie-triagegroep (158 dagen; spreiding 71-222; p=0,00084).
