Het is al langer bekend dat een dagelijkse dosis aspirine na de diagnose darmkanker een overlevingswinst kan geven van enige jaren. Onlangs publiceerde de onderzoeksgroep van dr. G.-J. Liefers, als oncologisch chirurg verbonden aan het Leids Universitair Medisch Centrum, in JAMA Internal Medicine een nieuwe analyse. Hieruit zou blijken dat de verbeterde overleving mogelijk afhangt van de tumorcelexpressie van HLA klasse I. “Het ging in deze retrospectieve studie om een relatief lage dosering en we stellen een nieuw verklarend mechanisme voor. Verder gaan we op basis hiervan een gerandomiseerde fase 3-studie starten bij ouderen.”
“Dat dit media-aandacht trekt, komt omdat zo’n dertig procent van de mensheid aspirine slikt en blijkbaar niet iedereen van onze eerdere studie op de hoogte was,” meent Gerrit-Jan Liefers. De studie werd drie jaar geleden geïnitieerd toen de Rotterdamse hoogleraar Experimentele Pathologie Ricardo Fodde de vraag neerlegde welke mogelijkheden er waren voor klinisch onderzoek naar de preventieve rol van aspirine bij darmkanker. Liefers: “Wereldwijd waren al diverse klinische studies over het aspirine-effect gepubliceerd, maar dat ging om kleine aantallen patiënten. Onze postdoc dr. Esther Bastiaannet, epidemioloog op het gebied van heelkunde en ouderengeneeskunde, wist het darmkankerincidentiebestand van het Integraal Kankercentrum Zuid (tegenwoordig IKNL, locatie Eindhoven) te koppelen aan een bestand van het PHARMO-instituut, een databank waarin medicatie- en ziekenhuisgegevens zijn opgeslagen van miljoenen Nederlanders. Zo kon een darmkankergroep worden geselecteerd aan wie ooit aspirine was voorgeschreven. Het bleek dat voorzetting of starten van aspirine na de diagnose coloncarcinoom inderdaad was geassocieerd met een overlevingswinst, een bevestiging die we in 2012 publiceerden in het British Journal of Cancer.1Deze publicatie vormt de basis voor een binnenkort te starten fase 3-studie bij ouderen, die dubbelblind zullen worden gerandomiseerd tussen aspirine en een placebo.”
Jacht op paraffineblokjes
Intussen publiceerde een Amerikaanse onderzoeksgroep in The New England Journal of Medicine dat aspirine met name preventief zou werken bij patiënten met in hun colorectale tumorcellen een mutatie in het PIK3CA-gen.2Deze bevinding wilde men graag bevestigd zien. “Onze promovenda Marlies Reimers heeft in het zuiden des lands alle paraffineblokjes verzameld van darmkankerpatiënten die tussen 2002 en 2008 waren geopereerd. Dr. Ronald van Eijk van de afdeling Pathologie wist vervolgens uit blokjes van 999 patiënten DNA voor een PIK3CA-mutatieanalyse te isoleren. Op weefselmicroarrays werd bovendien immuunhistochemisch de expressie geanalyseerd van prostaglandine-endoperoxidesynthase-2 (PTGS2, cyclooxygenase-2), een enzym in dezelfde signaaltransductieroute als PIK3CA, en van de klasse I humaan leukocytenantigenen (HLA-I). Het is bekend dat PTGS2-overexpressie geassocieerd is met een slechte prognose en dat het wordt geremd door aspirine. Ook is bekend dat na uitschakeling van de HLA-I-antigenen op tumorcellen, deze cellen gevoeliger zijn voor de celdodende activiteit van natural killercellen, omdat ze dan niet langer omgeven zijn door een beschermende mantel van bloedplaatjes. We vroegen ons daarom af of het aspirine-effect geassocieerd zou kunnen zijn met lage of afwezige expressie van HLA-I.”
Wat bleek: de correlatie met een PIK3CA-mutatie kon niet worden bevestigd, noch die met PTGS2-overexpressie. En opmerkelijk genoeg bleken juist HLA-I-positieve tumoren zeer significant gevoeliger te zijn voor het effect van aspirine.3 “Nu werkten we al enige tijd samen met dr. Ruth Langley van de Medical Research Council (MRC) in Londen, coördinator van een andere aspirinestudie die binnenkort van start gaat, en via haar kwamen we in contact met Italiaanse farmacogenetici die ons ervan wisten te overtuigen dat de door ons voorgeschreven 80 mg aspirine per dag te laag was om enige invloed te kunnen uitoefenen op celsignalering. De Amerikanen hadden in hun studie 325 mg gebruikt.”
Epitheel-mesenchymtransitie
De hypothese werd nu dat aspirine in hoge dosering weliswaar zou kunnen aangrijpen op celsignalering, bijvoorbeeld door PTGS2-remming, maar in lage dosering werkt via een heel ander mechanisme. Liefers: “Het is denkbaar dat circulerende tumorcellen (CTC’s) door een directe interactie met bloedplaatjes een metastatisch fenotype krijgen. Er zijn in-vitrostudies beschreven die laten zien dat die interactie een verhoogde activiteit induceert van zowel nucleaire factor-κB als tumorgroeifactor-β. Aldus krijgen CTC’s een meer mesenchymaal fenotype; ze worden minder zichtbaar voor T-cellen en het stimuleert metastasevorming. De interactie van tumorcellen met bloedplaatjes is afhankelijk van intact HLA-I en mogelijk remt aspirine de door die interactie opgewekte cellulaire signalen.”
Eén van de consequenties van deze verklaring is aldus Liefers dat dit mechanisme ook zou kunnen opgaan voor andere epitheliale tumorsoorten die CTC’s genereren. “Inderdaad blijkt dat aspirine ook een significant effect heeft bij mama-, oesophagus-, en maagcarcinoom.”
Aspirinestudie bij ouderen
De volgende stap is het in het najaar starten van de prospectieve landelijke fase 3-studie bij zeventigplussers met coloncarcinoom, in samenwerking met de Stichting Geriatrische Oncologie Nederland (Gerionne): de ASPIRIN-studie. “We hebben al veertig ziekenhuizen tot medewerking bereid gevonden. Het was trouwens lastig hiervoor subsidie te regelen. De farmaceutische industrie kan immers niets aan aspirine verdienen en voor het KWF Kankerbestrijding was de doelgroep minder interessant.” Uniek is nu dat een patiëntenvereniging, de Stichting voor Patiënten met Kanker aan het Spijsverteringskanaal (SPKS), het initiatief nam om subsidies te werven van het Innovatiefonds van de Zorgverzekeraars en van het Fonds Nuts Ohra. Het zal gaan om 1.588 patiënten die al of niet chemotherapie hebben gehad en die worden gerandomiseerd tussen vijf jaar lang 80 mg aspirine per dag of een placebo.
De bovengenoemde MRC-studie (Add-Aspirin-studie) gaat vijf jaar dagelijkse doses van 100 of 300 mg vergelijken met een placebo bij patiënten met tumoren in het maag-darmkanaal of borst- of prostaatkanker. Verder is er vanuit Singapore onder leiding van dr. John Whay-Kuang Chia een studie gestart die drie jaar dagelijks 200 mg aspirine versus placebo onderzoekt in Dukes C of hoog-risico Dukes B colorectale kanker (ASCOLT-studie). De onderzoekers van deze drie studies houden nauw contact en hopen te zijner tijd de uitkomsten in een meta-analyse te kunnen vergelijken.
Juist bij ouderen, met veel comorbiditeit, is toepassing van aspirine volgens Liefers zinvol. “Voor die kwetsbare groep is nauwelijks een alternatief voorhanden. Een HLA-I-analyse is dan wel gerechtvaardigd, want 0,7% van de zeventigplussers krijgt een bloedig CVA, maculadegeneratie of een darmbloeding.”
