Oudere patiënten met een diffuus grootcellig B-cellymfoom worden niet altijd volgens de standard of care behandeld uit vrees voor een aanzienlijk verlies van kwaliteit van leven. Op het Dutch Hematology Congress liet internist i.o. drs. Marjolein van der Poel (Maastricht UMC+) zien dat qua klinische relevantie de kwaliteit van leven van oudere patiënten niet slechter is dan van leeftijdsgenoten zonder diffuus grootcellig B-cellymfoom. Dat is echter wel het geval bij jonge patiënten.
De verwachting van KWF Kankerbestrijding is dat de incidentie van het agressief non-hodgkinlymfoom zal blijven stijgen. Het diffuus grootcellig B-cellymfoom (DLBCL) is de meest voorkomende vorm van dit lymfoom. Nadat bleek dat R-CHOP (rituximab; in combinatie met CHOP: cyclofosfamide, doxorubicine, vincristine, en prednison) ten opzichte van CHOP tot een betere klinische respons en totale overleving leidt, is de huidige standard of care voor DLBCL-patiënten R-CHOP.1 Toch wordt er regelmatig voor gekozen om, met name oudere patiënten, niet met R-CHOP te behandelen. Hiervoor worden een slechte performance status, comorbiditeit en het besluit van de patiënt om niet aan de studie mee te doen vaak als oorzaak genoemd. Zorgen over een mogelijk verlies van kwaliteit van leven door de behandeling met R-CHOP spelen hierbij een grote rol.
Van der Poel en collegae vergeleken de gezondheidsgerelateerde kwaliteit van leven (HRQOL) van DLBCL-overlevenden om een beter beeld te krijgen van de behandelingseffecten. Voor deze studie werden de patiënten ingedeeld in drie leeftijdsgroepen: 18-59, 60-75 en 76-85 jaar.2,3 Tevens werd de HRQOL van deze groepen vergeleken met die van gezonde personen met overeenkomstige leeftijd en geslacht.
DLBCL-patiënten werden geselecteerd uit de database van de Eindhovense Kankerregistratie. Via deze database werden onder andere gegevens verkregen over het ziektestadium en de primaire behandeling. Daarnaast werden onder meer psychosociale gegevens uit de PROFILES (Patient Reported Outcomes Following Initial treatment and Long-term Evaluation of Survivorship)-registratie gebruikt. In totaal werden van de 1.186 DLBCL-patiënten 363 patiënten uitgenodigd om een kwaliteit-van-levenvragenlijst van de EORTC in te vullen. Van hen reageerde 85%. De antwoorden werden vergeleken met die van gezonde personen geïncludeerd door het CentERdata-onderzoeksinstituut verbonden aan de Universiteit van Tilburg. De onderzoekers keken niet alleen naar significante verschillen tussen de groepen, maar richtten zich met name op verschillen met een grote of minder grote klinische relevantie.
Kwaliteit van leven jongeren
Uit het onderzoek bleek dat, volgens verwachting, de jonge DLBCL-overlevenden met betrekking tot fysiek functioneren, algehele kwaliteit van leven, eetlust en constipatie beter scoorden dan de oudste groep patiënten (alles p<0,05). Deze verschillen hadden echter geen of lage klinische relevantie. Daarentegen hadden jonge patiënten meer financiële problemen dan de patiënten van boven de 75 jaar. Dit verschil bleek wel klinisch relevant te zijn. Vergeleken met de normale populatie scoorden de drie groepen DLBCL-overlevenden over het algemeen significant slechter op het gebied van fysiek, cognitief en psychosociaal functioneren en hadden zij grotere financiële problemen. Hiervan hadden grotere financiële problemen, en lage scores in cognitief en sociaal functioneren van de jonge DLBCL-patiënten daadwerkelijk een klinisch relevantie. “De impact van het hebben van een DLBCL en de behandeling daarvan lijkt dus groter te zijn voor jongere patiënten dan voor oudere patiënten. Er moet dus niet al bij voorbaat vanuit worden gegaan dat een behandeling de kwaliteit van leven van ouderen wel erg zal verslechteren”, concludeerde Van der Poel.
Tijdens de discussie legde Van der Poel uit dat patiënten van boven de 85 jaar niet in de studie werden meegenomen, omdat men er destijds van uitging dat deze patiënten de vragenlijst waarschijnlijk niet zelf in konden vullen. Ze voegde daaraan toe dat deze patiënten tegenwoordig wel worden geïncludeerd. Verder werd gevraagd of het ontbreken van een aanzienlijk aantal verzonden vragenlijsten niet een dramatische invloed op de resultaten zou kunnen hebben, omdat deze vragenlijsten mogelijk waren blijven steken bij patiënten met een slechte kwaliteit van leven. Van der Poel gaf echter aan dat van deze geselecteerde patiënten geen gegevens werden verkregen, omdat een aantal ziekenhuizen had besloten om niet in de studie te participeren. Afsluitend merkte Van der Poel op dat de verkregen gegevens onvoldoende gedetailleerd waren om het effect van specifieke behandelingen op de kwaliteit van leven per lijftijdscategorie te onderzoeken.
